Your Time Is Important

To expedite your first office visit, kindly have your dental and medical information in advance of your appointment. We offer three options:

  1. Fill out your necessary patient information here on this website and send it to us electronically.
  2. Print your information if you prefer, and send it to our facsimile at 410-266-3850.
  3. Arrive 15 minutes prior to your appointment (click here for directions to the office) and we will assist you in filling out the necessary information.

Dental Plans

We accept most major dental plans with the exclusion of HMOs and PPOs. Our administrative staff will verify your coverage and will do everything we can to assist you with your insurance. Please review the information on the Dental Insurance form for all our arrangements on insurance or visit our Insurance section.

Necessary X-rays

We believe in only taking necessary x-rays. If you have current (less than 1 year old) x-rays from a previous dentist, please request that they be sent to Dr. Sheen's office prior to your initial appointment. Dr. Sheen uses a digital format, which is the latest in radiographic technology. In addition to reducing the radiation exposure by 85-90%, these films can also be sent electronically to a specialist or your insurance company. It is environmentally safe as no chemicals are used in the developing process.

Pre-medication

Please call the office if you require any pre-medication prior to your dental visits. A pre-medication might be necessary if you have a heart murmur, mitral valve prolapse or if you have had a prosthetic joint replacement. If uncertain, please check with your physician or orthopedic surgeon.

Determing Course of Treatment

Using all of our examination findings, together, we will develop a treatment program that will insure your mouth remains in optimum condition. Prior to undertaking any treatment program, we will discuss all findings and insure your questions are answered fully. Please also refer to the Financial Arrangements section for further familiarity with appointment and payment procedures.
