Dental Insurance
As a professional courtesy to our patients, we file your dental insurance. Dental insurance is not like Medical coverage and rarely covers the same percentage. The professional treatment you receive at Dr. Sheen's office is rendered to you, not the insurance company. You are responsible to us for the obligation of payment of treatment. Please understand that your insurance policy is a contract between you and your insurance company. Any problems of non-payment or a delay of payment are your responsibility.
Remember, dental benefits were never meant to determine your dental care; they are to assist the patient in the payment of your treatment choice.
When you call the office to schedule your visit, please do not hesitate to inquire about your dental insurance coverage. Have your company insurance information ready when you call for your appointment. In some cases insurance companies do not pay the provider (treating dentist), they only pay the patient. If this is the case, it is your responsibility to pay at each visit. We will file all forms for you.
Because we are a high tech practice, we will file some claims electronically, therefore a signature on file is required by all dental insurance companies. We must have a filled out insurance form to file your insurance for you. We will provide this necessary form at your first visit.
You are responsible for portions not covered by your policy on the day of service. For your convenience we take Visa/MC, AMEX, checks and cash.
Any insurance balance over 60 days old is delinquent and is your responsibility to pay.
Accident Insurance cases will be handled by the patient paying for the treatment at the time of service and your insurance or med-pay will reimburse you. Accident claims can take months to settle and we unable to wait for settlement.
We will always do our best to see that you receive full benefits. However, ultimate responsibility for payment is yours and financial arrangements must be defined prior to beginning dental treatment.
Dr. Sheen is not listed or does not accept any HMO's, workers compensation, or medical pay due to an accident.
Do You Take My Insurance?
As a courtesy to our patient's, we file all indemnity type insurance. Indemnity insurance is the type of insurance that allows you to go to the dentist of your choice. We do not belong to any HMO, PPO (Preferred Provider Option) or DMO (Dental Managed Care Option) plans. We do file PPO and DMO insurance. Please understand PPO/DMO typically will give you the option to select a provider outside your network. We recommend you contact your insurance carrier and confirm what penalties, if any, you will experience by going out of network. We have numerous patients with PPO/DMO coverage and most of them find there is only a minimal fee and it is well worth the difference. Remember, the choice is yours.
